0:00The last dive

On the eighteenth of June, 2023, five people climbed into a submersible called Titan, about three hundred and seventy-two miles southeast of St. John's, Newfoundland. They were sealed inside it, and the hatch was bolted from the outside. They were going down to look at the wreck of the Titanic, two and a half miles below them.

A submersible is not a submarine. A submarine can leave port and travel on its own. A submersible has almost no range of its own; it has to be carried out to the site and launched from a surface ship, and then it has to be recovered. Titan's surface ship was the Polar Prince.

About ninety minutes into the dive, at roughly ten forty-seven in the morning, Titan sent a short message to the surface. Six seconds later, the Polar Prince received its last signal. Then nothing. For the next four days, the world watched a search.

Eleven vessels and four aircraft covered more than twelve thousand square miles of ocean. There were reports of banging noises detected in the water, and those reports were carried on every news channel on earth, and for four days people allowed themselves to hope. There was never anything to hope for. Titan's hull had failed within that same minute, and the vessel had imploded, and hydrophones in the ocean had recorded the sound of it happening.

The five were Stockton Rush, sixty-one, the chief executive of OceanGate, the company that built and operated Titan, and who was piloting. Paul-Henri Nargeolet, seventy-seven, a French deep-sea explorer who had been down to the Titanic more times than almost anyone alive. Hamish Harding, fifty-eight, a British businessman and adventurer. Shahzada Dawood, forty-eight, a British-Pakistani businessman.

And his son, Suleman Dawood, who was nineteen. Four of them were paying passengers. OceanGate did not call them passengers. It called them mission specialists, and that phrase is not decoration.

It is one of the reasons this dive was legal, and we will come back to it. In August 2025, after a two-year investigation, the United States Coast Guard published its report. Its first sentence uses a word that investigators are extremely reluctant to use. It says five people died in what is considered to be a preventable tragedy.

other deep-diving submersibles are built. Nearly every crewed submersible that goes truly deep has a pressure hull - the sealed chamber that holds the people and keeps the ocean out - and that hull is a sphere, or a cluster of spheres, made of steel or titanium. There are two reasons. A sphere is the strongest possible shape against pressure pushing in from all sides.

And metal, when you overload it, tends to bend and yield in ways engineers can measure, predict, and inspect. Titan's pressure hull was neither of those things. It was a cylinder, roughly five inches thick, wound from carbon fibre composite, with a titanium dome capping each end. Carbon fibre composite is built up in layers, bonded to each other - think of plywood, or a stack of paper glued sheet to sheet.

Along those layers it is extraordinarily strong for its weight. Between them, it is much weaker. Remember that, because everything that follows depends on it. The shape and the material were not arbitrary.

A cylinder let Titan carry five people, where a sphere of comparable engineering would carry two or three. Five seats is what makes a paying expedition business possible. The commercial logic and the structural choice were the same decision. The destination was the most famous shipwreck in the world, lying at about three thousand eight hundred metres - a little over two and a third miles down.

At that depth the water presses on every square inch of the hull with roughly four thousand nine hundred pounds of force. The Coast Guard put the figure at the moment of the implosion at about four thousand nine hundred and thirty pounds per square inch. At that pressure there is no gradual failure, and at that depth there is no rescue. According to a 2023 customs declaration, Titan and the system used to launch and recover it were valued together at five point six million Canadian dollars - about four point two million US.

One more thing about Titan, because it is the single most repeated fact about this disaster and it deserves to be dealt with early. It was steered with a modified video-game controller. After the implosion, that controller became the symbol of the whole affair. It is worth being precise: commercial off-the-shelf controllers are used elsewhere in the industry, and no investigation concluded that the controller caused the loss of Titan.

Neither report says it. What killed the five people on board was the hull. There is a system that exists specifically to stop a vessel like this from being built badly, and Titan was deliberately kept outside it. It is called classification.

5:12The Choice Not To Be Certified

A classification society is an independent organisation that reviews a vessel's design before it is built, watches it being built, inspects it afterwards, and certifies that it meets a published standard. It is the marine equivalent of an aircraft being certified before it is allowed to carry passengers. It is not a rubber stamp; it is the mechanism by which the industry checks itself. According to the chair of the Marine Technology Society's committee on crewed underwater vehicles - the Marine Technology Society being the main professional body for this industry - there were roughly ten crewed submersibles in the world capable of reaching four thousand metres or deeper.

Every one of them was certified. Except Titan. Across the whole population of crewed submersibles, somewhere between ninety and ninety-five per cent are certified. OceanGate chose not to be, and said so publicly, arguing that classification was slow and that it stifled innovation.

Now the phrase from the beginning. Titan operated in international waters - outside the territorial sea of any country, where no single nation's rules automatically apply. It was launched from a Canadian-flagged support ship. It was run by an American company.

And the people who paid to be on board were designated mission specialists rather than passengers. Each of those four facts moved the operation a little further from the reach of any one regulator. That last one is not a technicality, and we do not have to guess at what it meant, because the Coast Guard's investigators put the question to two people who had been mission specialists themselves. They were aboard Titan on dive eighty-seven - a shallow test dive on the twelfth of June, 2023, six days before the vessel was lost.

One of them was asked to define the term in his own words. A passenger. A donation to research that came with a trip. That is what the designation was covering, described by a man who held it.

The National Transportation Safety Board - the American agency that investigates transport accidents, which conducted its own inquiry alongside the Coast Guard's - put this in flat language. It found that United States and international voluntary guidance, and United States small passenger vessel regulations, were insufficient to ensure that OceanGate followed established industry standards. Voluntary is the operative word. The main Coast Guard document covering this kind of operation, a circular numbered oh-five dash ninety-three, is guidance.

It is not a binding rule. There was nothing to enforce, so no one enforced anything. And the Coast Guard's own board went further than describing a gap. Among its findings it listed OceanGate's circumvention of United States laws and international standards, and a weak regulatory framework for submersibles.

The board was describing both a company that went around the rules and a set of rules easy to go around. In 2018 - five years before the disaster - thirty-eight people who design, build, operate and study crewed submersibles signed a letter to OceanGate. It came from the Marine Technology Society's committee on crewed underwater vehicles,

and it was not a vague expression of unease. It said this. Our apprehension is that the current experimental approach adopted by OceanGate could result in negative outcomes, from minor to catastrophic, that would have serious consequences for everyone in the industry. Catastrophic.

In writing. Five years early. Signed by thirty-eight specialists in a field small enough that they all know each other by name. The same year, the warning came from inside the company.

David Lochridge was OceanGate's director of marine operations - the person responsible for the safety of crew and clients during operations. In January 2018 he produced a report setting out his concerns about the carbon fibre hull, including flaws he had seen in it. He objected to the company diving Titan without first performing non-destructive testing: scanning the hull for internal defects without taking it apart, the way an ultrasound looks inside a body without cutting it open. OceanGate fired him.

Then the company sued him for disclosing confidential information. He counter-sued for wrongful dismissal, and in 2018 the two sides settled out of court. In September 2024, when the Coast Guard held its public hearing, Lochridge testified. He was asked whether, back in 2017, he had confidence in the way Titan was being built.

His answer was: no confidence whatsoever, and I was very vocal about that, and still am. That is the man the 2025 report would come back to. Its finding is that the absence of a timely investigation into his 2018 complaint was a missed opportunity for the government to intervene early. There is also the matter of what Stockton Rush said on the record while he was alive.

In a 2022 interview with the journalist David Pogue, Rush said that at some point, safety is just pure waste. In another interview he said that you are remembered for the rules you break. Those quotations are played endlessly now, and it would be easy to use them as a verdict. They are not a verdict.

A posture is not a cause. The reason they belong here is not that a man said reckless things; it is that the engineering record shows the posture was carried through into how the vessel was designed, tested, monitored, and kept in service. That record is what the rest of this is about. OceanGate numbered every outing Titan made.

On the fifteenth of July, 2022, it made the dive the investigators would later call dive eighty. During it, the people on board heard a loud bang. That much was public. What the Coast Guard investigation established afterwards is the part that matters.

12:08Dive 80

The sound was not only heard. It was recorded. Two different types of sensor fitted to Titan captured the same loud acoustic event. The vessel was carrying instruments whose entire purpose was to listen to its own hull, and on that dive, those instruments recorded the hull doing something a hull must never do.

What it was doing has a name: delamination. Those bonded layers of carbon fibre coming apart - separating from one another inside the material, where nobody can see it from the outside. The Safety Board's finding here is specific, and it is the hinge of this entire story. It found that the Titan pressure vessel likely sustained damage after it surfaced at the end of dive eighty, in the form of one or more delaminations, which weakened the pressure vessel.

Not on the final dive. At the end of dive eighty. In July 2022. Eleven months before anyone died.

And then the fact that is hardest to sit with. Between that dive in 2022 and the dives of 2023, no maintenance was performed on Titan's hull. The vessel that had recorded a loud acoustic event, built from a material that fails invisibly between its layers, was not repaired, was not rebuilt, was not scanned - and went back into the water. Titan carried a system OceanGate called real-time monitoring.

The idea behind it was genuinely interesting, and the company presented it as a safety feature. Acoustic sensors listened to the hull, and strain gauges measured how much it was flexing - a strain gauge being a small sensor bonded to a structure that registers how far the material stretches under load. Both ran continuously during a dive.

13:52The Data They Misread

The promise was that if the structure began to fail, the crew would know in time to drop weights and surface. The hull would tell you before it broke. The Safety Board's finding on that system is unambiguous. OceanGate's analysis of the real-time monitoring data was flawed, and because it was flawed, the company was unaware that Titan was damaged and needed to be immediately removed from service after dive eighty.

Read that carefully. The system detected it. The instruments worked. The reading of the data did not.

And the Coast Guard's board named the same thing from the other direction: it found the company had relied excessively on real-time monitoring to assess the condition of the hull, and then failed to conduct a meaningful analysis of what the system produced. And there is one more thing in the record that puts the whole of this in order. The Safety Board's report lists every dive Titan made. After the damage - after dive eighty, in July 2022, and the further damage after dive eighty-two - Titan made exactly one more deep dive that year: dive eighty-three, on the twenty-third of July, 2022, to two thousand nine hundred and fifty-four metres.

Then it stopped going deep. The next four dives, in May and June of 2023, reached eight metres, ten metres, ten metres and ten metres. Shallow checks in harbour water. So when Titan left the surface on the eighteenth of June, 2023, and headed for three thousand three hundred metres, it was the first time in eleven months that the damaged hull had been taken back into the deep ocean.

Behind the misreading sits something more fundamental, and it is what the Safety Board built its probable cause on. OceanGate did not adequately test Titan. Because it did not adequately test it, the company did not know how strong the hull actually was - and the Board's assessment is that the real strength and durability were likely much lower than the company's own target. They were operating a vessel whose limits they had never established.

The Coast Guard's board added that no analysis was ever conducted to understand the expected cycle life of the hull: how many dives it could be expected to survive. That ignorance extended past the hull itself, into how Titan was handled between dives. The Board found the company was unaware how certain operational changes - including the conditions in which Titan was stored, and the way it was towed - could affect the integrity of the pressure vessel. Between expeditions, Titan was kept outdoors.

One of the figures in the report is an overhead photograph of a parking lot in St. John's, Newfoundland, where the submersible was stored. The damage did not stop at dive eighty, either. The Safety Board found that after dive eighty-two, Titan sustained additional damage - of unknown origin, which is the report stating plainly that it does not know what caused it - and that this further deteriorated and weakened the pressure vessel.

So the vessel that entered the water on the eighteenth of June, 2023 was carrying at least two separate rounds of structural harm: the delaminations from July 2022, and something after dive eighty-two that no one has been able to identify. That dive on the eighteenth of June was Titan's eighty-eighth. The eighteenth of June, 2023. Titan is launched from the Polar Prince, three hundred and seventy-two miles southeast of St.

John's. It begins its descent. Communication with the surface is by short text message, not voice. At around ten forty-seven in the morning, roughly ninety minutes into the dive, Titan sends one.

It reads: dropped two wts. Two weights. Dropping weights is what you do to become buoyant - to go up.

The family of one of the men on board has argued in a lawsuit that this message indicates the crew knew something was wrong and were trying to abort the dive. That interpretation has not been established as fact, and neither report asserts it. What is on the record is the timing. Six seconds after that message, the Polar Prince received Titan's last ping.

The final communication came at a depth of about three thousand three hundred and forty-one metres. Communication and tracking were lost at three thousand three hundred and forty-six. Titan was less than five hundred metres from the Titanic. The Coast Guard's board identified the initiating event, at approximately ten forty-seven that morning, as the loss of structural integrity of Titan's carbon fibre hull.

The layers inside it had already come apart. Under that pressure, a weakened section of the cylinder gave way inward - a local buckling failure, the Safety Board called it, meaning a collapse that begins in one place rather than across the whole structure at once. And once any part of that hull began to move inward, nothing could stop it. The collapse propagates at a speed measured in thousandths of a second.

The five people on board were exposed to about four thousand nine hundred and thirty pounds per square inch, and the Coast Guard's finding is that their deaths were instantaneous. Whatever else is true of this disaster, they did not experience it. And because sound carries extraordinarily well through deep water, the event itself was recorded. Hydrophones picked up the signature of the implosion.

That recording was entered into the investigation's public record. That was the first morning. Before the search began. Titan was due back on the surface at three in the afternoon.

It did not appear. What followed was one of the largest and most closely watched search operations in recent memory. The numbers are in the Coast Guard's report: eleven vessels and four aircraft searched twelve thousand one hundred and forty-five square nautical miles of ocean, looking for survivors and debris. During it came reports of banging noises detected in the water, described in some coverage as possibly rhythmic - possibly someone striking the hull.

20:25Four Days Of Listening

Those reports drove the hope of that week around the world. They were not Titan. Titan had imploded on the first morning, before the search began. The United States Navy had in fact detected an acoustic anomaly consistent with an implosion at around the time contact was lost.

On the twenty-second of June, at about twenty to ten in the morning, a remotely operated vehicle - an unmanned machine controlled from the surface by cable - called Odysseus, rated to six thousand metres, operated by Pelagic Research Services and deployed from a ship called the Horizon Arctic, found Titan's aft tail cone and other debris on the seafloor. That discovery confirmed what the acoustic record had already established four days earlier. And there is a finding about that search which is easy to miss. OceanGate had not notified search and rescue authorities that the expedition was taking place.

The Safety Board found that had the company followed the existing guidance for emergency response plans - that circular, oh-five dash ninety-three - it would likely have had emergency response assets standing by, and Titan would likely have been found sooner. The Board is careful and honest about what that would and would not have meant. A rescue was not possible in this case. Nobody could have been saved, at that depth, from that event.

But four days of searching, and the hope that went with it, were spent looking for something that could have been located far earlier. In August 2025, the United States Coast Guard published the report of its Marine Board of Investigation. Three hundred and thirty-five pages. Its opening sentence reads: five people died when the commercial submersible Titan imploded on the eighteenth of June, 2023, in what is considered to be a preventable tragedy.

The board named the primary causal factor as OceanGate's failure to follow established engineering protocols for safety, testing, and maintenance of their submersible. Then it listed eight primary causal factors, and they read less like a list than a sequence. The design and testing

processes did not adequately address fundamental engineering principles. No analysis was conducted to understand the expected cycle life of the hull. The company relied excessively on real-time monitoring, and then failed to analyse meaningfully what that system produced. It kept using Titan after a series of incidents that compromised the hull, without properly assessing or inspecting it.

The winding, the curing, the gluing, the thickness, the manufacturing standards - all introduced flaws that weakened the hull. It failed to investigate in detail after those incidents. And it failed to conduct preventative maintenance on the hull, or to protect it from the elements during the long offseason layup before the 2023 expedition. That last one is the parking lot.

The board then listed what it called other contributing causal factors, and here it stops describing engineering and starts describing a company. OceanGate's safety culture and operational practices were critically flawed, and at the core of those failures were glaring disparities between the company's written safety protocols and its actual practices. Senior leaders fostered a culture in which mounting financial shortfalls, customer expectations and operational demands were allowed to override the authority of the mission director responsible for each dive. And there was no comprehensive regulation for crewed submersibles and vessels of novel design in the first place.

And one finding about the chief executive. The board found that Stockton Rush made sustained efforts to misrepresent Titan as indestructible, on the basis of safety margins that were never confirmed and a claimed conformance with advanced engineering principles - and that in doing so he provided a false sense of safety to passengers and to regulators. There is one more finding, and it belongs to the man from chapter three. The board found that the absence of a timely investigation by the Occupational Safety and Health Administration - the American workplace-safety regulator - into a 2018 OceanGate whistleblower complaint, combined with a lack of effective coordination between that agency and the Coast Guard over seaman protection procedures, was a missed opportunity for early government intervention.

The Safety Board stated its probable cause in the flat language these documents use. The probable cause of the hull failure and implosion of the submersible Titan was OceanGate's inadequate engineering process, which failed to establish the actual strength and durability of the Titan pressure vessel, and resulted in the company operating a carbon fibre composite vessel that sustained delamination damage - damage subsequently made worse by additional damage of unknown origin - resulting in a damaged internal structure that led to a local buckling failure. Strip it down and it says this. They never established how strong it was, so they could not tell when it stopped being strong enough.

Between them, the two investigations made recommendations, and reading them is the clearest way to see what was missing. The Coast Guard's board made fourteen safety recommendations and two administrative ones. The first is to establish an industry working group to review and update that 1993 circular. Others close the gap that let Titan exist at all - including ending the practice of designating submersibles as oceanographic research vessels, a classification the board found was only ever written for surface research ships and which could be used to sidestep passenger-vessel inspection.

One requires that a submersible be able to talk to the surface by voice at the maximum depth it is authorised to reach. And the fourteenth requires operators to notify the local Coast Guard captain of the port before diving, with a dive plan and an emergency response plan capable of reaching the depth they intend to work at. That is the recommendation that exists because nobody knew Titan was going down that morning. The Safety Board added four of its own, to the Coast Guard: commission a panel of experts to study how these vessels are operated, including the competency of the people on board and whether everyone aboard should be treated as a passenger regardless of what the operator calls them; then write actual United States regulations for pressure vessels that carry human beings, and if the Coast Guard does not currently have the legal authority to do so, obtain it; revise the 1993 circular; and propose to the International Maritime Organization that the relevant international rules be made mandatory rather than advisory.

Read together, those recommendations amount to an admission. The framework that was supposed to prevent this did not exist in any binding form - and at the time the reports were published, it still did not. Titan made eighty-seven dives before its last one. The hull had been recording what was happening to it the whole time.

On dive eighty, in July 2022, it recorded the moment it was hurt, and that recording sat in the company's own data. Eleven months later, five people were sealed into it and taken two and a half miles down. Stockton Rush. Paul-Henri Nargeolet.

Hamish Harding. Shahzada Dawood. Suleman Dawood, who was nineteen years old. The Coast Guard's word was preventable, and the ordinary meaning of that word is the point.

Nothing new had to be discovered. No technology had to be invented. No one needed special insight to see it coming. The information already existed.

It had been written down, by instruments, on the hull's own record - and it was read wrong.
